Before your first appointment: Balance Institute of Indiana prides itself on provided the best quality and individualized care before your walk through our door. Prior to the appointment please print you Client forms located on this website. Bringing these completed forms to your initial visit will allow more quality time for you to spend without our expert therapists to aid in assisting you to a quick recovery. Thoroughly complete your paperwork in its entirety. If you are unable to print and complete these forms, plan on arriving at least 15 minutes prior to your scheduled appointment to complete the necessary information.
Your first appointment: Most first visits include an initial evaluation with a Physical Therapist. During your initial evaluation, you will meet with a Physical Therapist that will review the information you provide regarding your medical history and thoroughly examine you. This provides the means to accurately reach a treatment diagnosis and develop a customized treatment plan. The therapist will communicate their findings in details, make recommendations and work with the patient to develop an individualized specific treatment program. The therapist will design a specific program of exercise and activities for the patient to follow through with between visits to promote the ability to move, reduce dizziness, restore function and prevent disability or further injury. You are encouraged to ask any questions to help you comprehend all that involves your plan of care. It will range from 45 min to 1 hour in length.
Testing appointments: Depending on the type of testing you will be receiving, you will have specific instructions on how to prepare. Most vestibular testing recommends ALL vertigo/dizzy medicine and/or sleeping aids, and anti- histamines be stopped at least 24 hours prior to appointments. Refrain from caffeine and excessive sugar consumption. Provide any old hearing exams done prior. Please make sure your ear canals are free from wax. Testing appointments range from 30 minutes to 1 ½ hours. VNG patients are encouraged to have a driver available as this test can make you dizzy. All testing is non-invasive.
Follow up Visits: Be prepared to work! Most returning patients are expected to comply with their home exercise program as prescribed by their physical therapists. They must keep their scheduled appointments weekly and results must be achieved in order to continue. Patient’s typically return for 30 – 45 minutes appointments weekly, 1-3 x week depending on their individualized needs. You will work closely with your therapist one-on-one. We take this approach because we know the importance of the individualized care for successful rehabilitation. When all treatment goals have been achieved, the PT will create a comprehensive long term plan tailored for your specific needs to assist with maintaining your optimal health goals.
Payment for Services: It is an insurance requirement that all co-pays, co-insurances, deductible and all out of pocket costs are due at time of service. We will bill all appropriate insurances and submit claims forms on your behalf. As a courtesy to our patients we verify all primary insurance carriers to find out what your responsibility of payment is due for each service. Please note however, our process of verifying coverage in no way guarantees payment for our services and is ultimately your responsibility to know your insurance policies as it relates to coverage. You will be held liable for the payment of services. If for any reason you are not able to provide payment at time of services, our billing offices is available to assist you in developing a reasonable plan for payment
